As each participating facility's grant funding becomes depleted, we will customize and introduce to facility management the fee-based bbusiness model that will provide a seamess transition to continuity of care.
With the new model finalized, it is important to reconnect with the local participating physicians to explain the new model and reassure them that while the operational infrastructure will change, the actual programs and programming they helped create will remain as designed.
Before the last grant-funded programs conclude it is important to introduce the new operational fee-based model to the participants, and reestablish their individual committment to their personal wellness goals and the reduction or minimization of their specific disease.
Work with facility management, local physicians and program trainers to establish a solid game plan for marketing to and managing the relationships with program participants to optimize program renewal into the new model and minimize attrition.
Re-train and refresh all facility management and trainers on the software, paperwork, assessment equipment and manditory data collection within each program. Also review the best verbiage to use when presenting the new model to hesitant participants and other interested members.
Promote the new medically directed programming through focused and specifically worded local marketing, in-house social media, carefully selected brand ambassadors, internal signage, facility membership word-of-mouth, and referrals from participating physicians.
Enroll and schedule program participants based on medical condition(s) and individual program availability, adhering to all established participant parameters, noting any restrictions, and ensuring that all pre-program participant forms are properly and completely filled out, double-checked, and processed.
Implement all medically directed programs, measure and record all designed participant metrics, and instruct all participants on the use of our unique “Participant Portal” where each participant can log on and review a visual representation of his/her personal program progress towards desired goals.
Working in concert with facility management, program training staff and (if available) the referring local physicians, our leadership team will facilitate understanding and analysis of program data to determine efficacy, indicate potential program adjustments, and guide local marketing efforts.
By participating in safe and effective workouts that address each participants' wellness goals, and with metrics to show progress towards those goals, most participants will continue program after program because better health and a reduction of those diseases has no price tag and immense personal value.
